건성안 치료에 대한 IPL과 PRP의 효과 비교 분석 보고서

1. 서론

 

건성안 질환은 눈물막의 항상성 상실, 안구 표면 증상, 눈물막 불안정성, 고삼투압, 안구 표면 염증 및 손상, 신경 감각 이상을 특징으로 하는 다인성 질환으로, 성인 인구의 상당 부분을 차지하는 흔한 안구 표면 질환입니다.1 건성안의 증상은 경미한 자극감부터 심한 통증, 시력 저하까지 다양하게 나타나 삶의 질에 상당한 영향을 미칠 수 있습니다.2 건성안은 눈물 생성 부족(수성 눈물 결핍 건성안, ADDE)과 눈물 과도 증발(증발성 건성안, EDE)을 포함한 여러 하위 유형으로 분류되며, 마이봄샘 기능 장애(MGD)는 EDE의 주요 원인 중 하나입니다.3 쇼그렌 증후군과 같은 기저 질환은 건성안 발병에 크게 기여할 수 있습니다.9

기존의 건성안 치료법은 주로 인공 눈물, 항염증제, 눈물점 폐쇄와 같은 보존적 관리에 중점을 두었지만, 일부 환자, 특히 난치성 건성안 환자에게는 이러한 치료법의 효과가 제한적일 수 있습니다. 이러한 한계를 극복하기 위해 강렬 펄스 광선(IPL)과 혈소판 풍부 혈장(PRP) 치료와 같은 새로운 치료법이 등장하여 난치성 건성안 환자에게 잠재적인 해결책을 제시하고 있습니다. 본 보고서의 목적은 현재 연구를 기반으로 건성안 치료에 있어 IPL과 PRP의 효과를 종합적으로 비교 분석하는 것입니다.

 

2. 강렬 펄스 광선(IPL) 치료

 

IPL 치료는 515~1200nm 파장의 넓은 스펙트럼의 빛을 사용하여 특정 발색단을 표적으로 하는 광 기반 치료법입니다.9 건성안 치료에서 IPL의 주요 작용 기전은 마이봄샘 주변의 염증을 줄이는 것입니다. IPL은 비정상적인 혈관 확장증을 응고시켜 염증 매개체의 접근을 감소시키고 마이봄샘 기능을 개선하는 것으로 여겨집니다.14 또한 IPL의 열 효과는 굳어진 마이봄을 부드럽게 하여 분비를 촉진하고 눈물막 안정성을 향상시킬 수 있습니다.14 일부 연구에서는 IPL이 눈꺼풀 가장자리의 세균 과증식과 데모덱스를 감소시키는 데 도움이 될 수 있다고 합니다.14 IPL은 또한 IL-6, IL-17a, IL-1 및 PGE2와 같은 전염증성 마커를 하향 조절하는 잠재력을 보여주었습니다.14

3. 혈소판 풍부 혈장(PRP) 치료

 

PRP 치료는 환자 자신의 혈액에서 추출한 혈소판 농축액을 활용하는 혈액 유래 치료법입니다. PRP에는 혈소판 유래 성장 인자(PDGF), 상피 성장 인자(EGF), 형질 전환 성장 인자-베타(TGF-β), 혈관 내피 성장 인자(VEGF), 신경 성장 인자(NGF) 및 인슐린 유사 성장 인자(IGF)를 포함한 다양한 성장 인자가 풍부합니다. 이러한 성장 인자는 안구 표면의 세포 증식, 이동, 분화 및 상처 치유를 촉진합니다.4 PRP에는 또한 항염증 특성을 가진 사이토카인과 단백질이 포함되어 있으며 4, 염증성 사이토카인을 중화하고 미생물 침입을 방지하는 데 도움이 될 수 있습니다.36

PRP 치료의 임상적 효과는 다양한 연구에서 입증되었으며, 눈물막 안정성(TBUT) 개선, 안구 표면 손상 감소(OSS), 주관적 증상(OSDI 점수) 완화에서 긍정적인 결과를 보였습니다.4 PRP 점안액 치료 후 TBUT가 유의하게 증가한 것으로 나타났으며 4, PRP 주사는 중증 건성안 환자의 TBUT를 개선하는 데 효과적인 것으로 밝혀졌습니다.10 PRP는 각막 염색 점수(OSS)를 유의하게 감소시켜 안구 표면 손상을 개선하는 것으로 나타났으며 4, 각막 궤양 및 지속적인 상피 결손 치료에 효과적인 것으로 입증되었습니다.4 PRP 치료는 또한 OSDI 점수를 유의하게 감소시켜 주관적인 건성안 증상을 완화하는 것으로 나타났습니다.4

4. IPL과 PRP의 비교 분석

5. IPL과 PRP 병용 치료

6. 결론

 

결론적으로 IPL은 특히 MGD로 인한 증발성 건성안에서 눈물막 안정성과 마이봄샘 기능을 개선하는 데 효과적인 치료법입니다. PRP는 중등도에서 중증 건성안, ADDE 및 EDE를 포함한 다양한 유형의 건성안 치료에 광범위한 효능을 보이며, 쇼그렌 관련 건성안 및 안구 표면 치유에 대한 가능성을 보여줍니다. PRP는 더 높은 성장 인자 농도로 인해 종종 자가 혈청보다 우수한 것으로 간주됩니다. IPL과 PRP를 직접적으로 비교한 연구는 제한적이지만, 일부 연구에서는 특히 쇼그렌과 같은 특정 하위 유형에서 PRP의 잠재적인 이점을 시사합니다.
